Courtesy vendor forms

I will be starting this year with a new courtesy form and I am really hoping everyone will participate. This is for each couple to fill out so I know which vendors covered what in each wedding. It will be a quick, even over email, list of each vendor. Some I will know while others I will not. I know, I know, yet another form but I really hope everyone participates. I think many times we as vendors are not sure who made the cake, created the flowers or did the make-up and can miss each other completely in all the action of the day. And ultimately the photographer is the end result that showcases, what is most of the time, temporary work. I believe in giving and sharing back information and helping out other vendors at no charge. If a "vendor" happens to be a friend and doesn't want the referral business than that is fine and it will be kept private in the public listing as "a family friend".
I really need to catch up with the times and showcase everyone's beautiful work professionally. If anyone already has a form for this or made this process easier than pulling teeth, please do share! I would love some input and insight on this topic.
